Patient Registration
You may register at Handbridge Medical Centre as a new patient if your postcode begins with one of the following as you are in our practice area;
CH1 1, CH1 2, CH4 7 & CH4 8
If your postcode does not begin with any of these, then unfortunately, at this time we would be unable to register you as you are outside of our practice area, please click here to take a look at NHS choices for alternative practices who may register you. Please keep in mind your nearest practice may be in Wales, for Welsh surgeries please click here.
If you decide to register.
Please allow us 5-10 working days to process your registration (during busy periods this may take us longer), if you need to be registered quicker for emergency care please contact our reception team, and given we have the capacity we will assist you as best we can. Remember your previous practice still holds a duty of care to you even if you are currently out of their area so they may be able to assist with prescriptions and such. Also please be aware it often takes sometime for records to be forwarded from your former practice, therefore it would be wise for you to ensure you have a couple of weeks worth of medication, if needed, from your previous practice. Appointments will not be offered to unregistered patients.
Registrations will not be accepted without a registration form, lifestyle questionnaire, and signed patient contract, agreeing to our practices policies. These forms are available to download below. Or you can submit the pre-registration forms online to us by selecting the tab at the top of this page. However all new patients are required to attend the surgery in person before your registration is accepted so we can view PROOF OF IDENTITY and PROOF OF CURRENT ADDRESS. Any pre-registrations forms where we have not seen accompanying proof of ID/ADDRESS are discarded after 1 month.
If you have asthma, diabetes, COPD or a form of heart disease, once registered, we will contact you to make an appointment with our specialist nurse.
We do not routinely ask new patients to see a GP, however if you would like to arrange an appointment for this you are welcomed to. Our Health Care Assistant also can accommodate general health checks for our new patients.

The NHS is introducing a Summary Care Record to be used in emergency care. This will detail any medication you are on as well as allergies and bad reactions to medications. This information will only be available to health care staff treating you. Further information is available from reception or you can visit the following website www.nhscarerecords.nhs.uk Should you decide you do not want to be included in this initiative there are "opt out" forms available at our reception desk.
